We are looking for a RN Triage Nurse to join our team in Pediatrics and work collaboratively with healthcare professionals, and administrative staff to deliver patient-centered care.

Key Responsibilities:

  • Be the system navigator and point of contact for patients and families, providing patients and families direct access for asking clinical questions and raising concerns
  • Work under direction of providers to communicate laboratory, radiology and other test results to patients/families
  • Receive incoming telephone calls from patients and families; resolve patient needs and issues whenever possible, including triage of acute conditions
  • Receive requests for medication refills; complete medication refills that meet criteria established by HealthFirst Bluegrass Medication Refill Procedure.
  • Provide relevant self-management support for patients with chronic illnesses as identified by clinical teams
  • Work collaboratively with other team members to provide care management services for high risk patients

· Assess the severity of the patient’s health concerns using protocols and algorithms in a manner that provides patient dispositions to emergency care, clinic provider appointments, and triage nurse clinical instructions within the scope of the licensed professional.

· Manage nurse-directed patient telephone calls and calls from walk-in patients requesting health care services.

· Evaluate and assess patient’s health care needs based on patient’s signs and symptoms utilizing established triage protocols and guidelines.

· Obtain appropriate input and direction from the provider when needed to determine best clinical course of action and recommendations for specific patient health care needs.

· Accurately document symptoms/complaints, nursing assessment, advice provided, triage plan and follow up in the patient’s electronic medical record for all encounters, phone and face to face.

· Follow policies, procedures, and protocols to ensure consistency and departmental effectiveness as well as improve health care outcomes of patients/callers and their access to appropriate health care.

  • Participates in Quality Improvement and other activities, as assigned
  • Participates in Team Huddles as needed
  • Tracks and does follow-up with patients when they obtain services outside the practice like emergency rooms, hospitals and other health care facilities
  • Collaborates with other members of the Care Team to coordinate care and provides care management activities including implementation of the patient’s care plan
  • Educates patients over the phone and in person regarding their plan of care and assists them in achieving goals identified in their care plan.
  • Identifies and refers patients having problems related to the health care system, financial or social barriers (e.g., transportation services or prescription assists) and/or needing to be linked with community resources to Community Health Worker
  • Assists with data collection and generation of patient registry reports as needed
  • Ensures and/or remains in compliance with local, state, and federal regulation
  • Completes other duties may be assigned.

Qualifications:

· Registered Nurse and Nursing Diploma with three years of experience as a nurse or other health related position OR

· Associate’s Degree in Nursing (ADN) with five years of experience as a nurse or other health related position OR

· Bachelor’s Degree in Nursing (BSN) with one year of experience as Nurse or other health related position OR

· Master’s Degree in Nursing or other related field plus one year of nursing experience.

· Other acceptable relevant experience may include: other health related position, working with patients with chronic conditions, care coordination and care management in a medical setting, and managing competing priorities.
